One MTP connector at one end, four duplex LC connectors at the other: this 5m OM4 fanout joins MTP equipment to LC ports. MTP is a high-performance version of the MPO connector and mates with standard MPO ports.
A common use is breaking out a 40G or 100G QSFP port into four 10G or 25G SFP connections, or linking an MPO cassette to LC equipment.
The fibre is high-bandwidth 50/125 multimode fibre that runs 10 Gigabit up to 400 metres and 40/100 Gigabit up to 150 metres.
